Purpose:

The purpose of this policy page is to outline the guidelines and procedures for appointments, cancellations, patient responsibilities, and financial policies at Humanity Health Center. These policies are established to ensure efficient operations, maintain patient satisfaction, and provide clarity on expectations for both patients and staff members.

Scope:

This policy applies to all patients utilizing services provided by Humanity Health Center, including but not limited to scheduling appointments, adhering to cancellation policies, providing necessary documentation, and understanding financial obligations.

Responsibility:

It is the responsibility of both patients and staff members to adhere to the policies outlined in this document. Staff members are responsible for communicating these policies effectively to patients and ensuring compliance. Patients are responsible for understanding and abiding by the policies, as well as providing accurate information and adhering to financial obligations.

General Policy Statement:

  • To schedule an appointment, please TEXT or call us at 702-434-8880. We kindly ask for a 24-hour cancellation notice for scheduled appointments.
  • If you’d like to set up an appointment, you may call us at 702-434-8880 or text us at the appropriate location where you’d like to be seen.
  • In the event of a missed appointment without prior cancellation, a fee of $25 will be incurred.
  • For new patients, please bring your ID, insurance card(s), medication list, and any available medical records to your appointment. We recommend arriving at least 15 minutes early to complete necessary paperwork.
  • Patients with HMO plans must contact their insurance provider to change their primary care physician to our providers before their scheduled appointment and provide us with the reference number.
  • For uninsured patients, the first visit fee is $120, followed by subsequent visits at $80 each.
  • FMLA applications incur a $40 fee and require a minimum of two visits; caregivers applying for FMLA must be present during the visit.
